Physical Therapist Skilled

Job Description

We are seeking a Physical Therapist to join our Therapy team! You will plan physical rehabilitative programs to improve patient quality of life.

• Collaborate with other team personnel to achieve well-rounded care
• Provides skilled physical therapy in accordance with the physician’s plan of care
• Assists the physician in assessing the client’s functional level by applying appropriate tests and assists in the development and revision of the plan of care
• Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care
• Observes, records, and reports the client’s reaction to treatment and any change in the client’s condition to the physician and/or client care manager and other staff involved
• Evaluates, and regularly reevaluates, the rehabilitation needs of the client; initiates, develops, implements and makes necessary revisions of the client’s plan of care
• Conducts OASIS assessments as appropriate
• Supervises physical therapy assistants as assigned to license in accordance with licensing board requirements, payer requirements and agency policies
• Develops, prepares and maintains individualized client care progress records with accuracy, timeliness and according to policies
• Submits accurate documentation within 24 hours of visit

• Previous experience in physical therapy
• Ability to handle physical workload
• Ability to build rapport with patients
• Excellent written and verbal communication skills
• Strong leadership qualities

• Licensed as a physical therapist in the state
• Minimum of 6 months experience as a physical therapist
