Quick Facts

- Shift (type/length): Monday - Friday 40 hours/week 8a-5p
- Employment Type: Contract with options to extend until 2030
- Experience: 1 year of community work experience whether a paid position or volunteer
- MUST be bilingual (English & Spanish)
- Pay: $19.71/hour
- 11 Federal Holidays paid out
- 10 vacation days per year that begins accruing at 90 days
- 11 sick days per year that begins accruing day 1

- Provide community-based services tailored to the needs of high-risk and underserved populations and their families
- Build trusting relationships and help identify barriers to accessing health care and community services
- Explain program purposes and services in a culturally appropriate manner
- Support individuals in understanding health management plans, care plans, and instructions to help them reach goals
- Document activities, service plans, and outcomes in accordance with established policies and procedures
- Coordinate communication and services between individuals and providers/community partners
- Maintain and expand a database of community resources and assist with scheduling and applications for eligible programs
Required
- High School Diploma or equivalent
- 1+ year of community work experience (paid or volunteer)
- Strong organization, documentation, and communication skills (verbal and written)
- Computer skills, including Word, Excel, and database software used to track outputs
- Ability to work effectively with culturally diverse individuals and communities
As a Community Health Worker, you will be an integral part of an interdisciplinary health team, helping patients understand their care, connect to community services, and set achievable health goals. This role emphasizes advocacy, care coordination support, and consistent communication with the care team to promote quality outcomes and patient satisfaction.
