Days: 8 hours, Monday to Friday. Times are flexible as long as they are consistent and communicated with the Director of Rehabilitation.

Job Description

We are seeking a dedicated Occupational Therapist to effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders. As an Occupational Therapist, you will develop appropriate plans of care that align with all regulatory and clinical practice standards.

Responsibilities
  • Screen and evaluate patients to identify cognitive and physical functioning disorders.
  • Develop and implement individualized treatment plans using standardized assessments and evidence-based practices.
  • Provide rehabilitative treatment interventions including ADL retraining, therapeutic activities, and exercises.
  • Monitor patient progress and adjust treatment plans based on outcomes and evaluations.
  • Consult and provide guidance to patients, families, caregivers, and other service providers.
  • Maintain clear communication with supervisors and other health team members regarding patient care.
  • Document all clinical and regulatory information within electronic systems and maintain accurate patient records.
  • Participate in required meetings, trainings, and conferences to ensure compliance and enhance skills.
  • Supervise and support occupational therapist assistants, rehabilitation aides, and students as necessary.
Required Experience / Certifications / Licensure
  • Minimum of 1 year of OT experience is required.
  • Experience in Long-Term Care (LTC) or Rehabilitation settings is required.
  • Active Occupational Therapist license in the state of practice is required.
  • New Graduates with Level II fieldwork experience will be considered.
