Work Schedule: Monday – Friday, Primary Hours 8:00 a.m. – 5:00 p.m.
Job DescriptionThe LPN Patient Care Coordinator ensures that medically complex and specially‑sized Sooner Care members are effectively managed by coordinating access to medical care, community resources, and social services. The role includes facilitating care management activities, collaborating with intra‑ and inter‑agency personnel to ensure program adherence to state and federal guidelines, resolving member issues, contributing to case management decisions, and advocating for vulnerable populations while ensuring compliance with OHCA policies.
Responsibilities- Coordinate and facilitate access to medical care for medically complex/special health care needs members identified through various referral sources.
- Facilitate access to community resources and social service agencies within Sooner Care program parameters.
- Contribute to Sooner Care member care plan development and document care‑management activities using the clinical case management system.
- Participate in case staffing meetings and collaborate with other agency staff to ensure coordination of efforts.
- Interface verbally and in writing with members, families, and providers to resolve issues, acting as a Sooner Care Care Management resource.
- Collaborate with the interdisciplinary team to evaluate service appropriateness and contribute to care plan development based on clinical protocols.
- Other duties as assigned.
This position does not supervise.
QualificationsEducation and Experience
- Possession of a valid permanent Oklahoma license as a licensed practical nurse plus one year of practical nursing experience, OR
- Licensed Practical Nurse License recognized by the Enhanced Nurse Licensure Compact (eNLC) plus one year of practical nursing experience.
Preference May Be Given To Candidates With
- 2 or more years of practical nursing experience as a licensed practical nurse
- Experience with obstetric care
- Experience with aged, blind or disabled populations
- Experience with Medicaid/Sooner Care
- Related professional certifications (other than BLS/ALS/PALS)
- Experience with MS, Excel, Access, Power Point
- Bilingual
