Full-Time | PM Shift | 4/2 Schedule Join Our Exceptional Nursing Team Moraga Post Acute is seeking a compassionate, dependable, and clinically skilled Registered Nurse (RN) to join our dedicated nursing team on a full-time PM shift with a 4/2 schedule. This is an excellent opportunity to become part of a collaborative team that is committed to delivering outstanding care in a supportive post-acute setting.

As an RN, you will play a vital role in overseeing resident care, leading the nursing team during your shift, and ensuring our residents receive the highest quality clinical services while maintaining their dignity, comfort, and well-being.

Key Responsibilities Provide skilled nursing care in accordance with individualized care plans

Administer medications, treatments, and physician orders accurately and safely

Assess residents' conditions and promptly identify and report changes

Supervise and support LVNs and Certified Nursing Assistants (CNAs)
Maintain accurate and timely clinical documentation

Collaborate with physicians, therapists, and the interdisciplinary care team Assist with admissions, discharges, assessments, and care planning

Ensure compliance with all federal, state, and facility regulations

Promote infection prevention, resident safety, and quality care initiatives

Foster a positive, respectful, and resident-centered environment

Qualifications

Current, active California Registered Nurse (RN) license

Strong clinical assessment and critical thinking skills

Excellent communication and leadership abilities

Ability to thrive in a fast-paced skilled nursing environment

Previous experience in post-acute, skilled nursing, or long-term care preferred, but not required

Passion for providing compassionate, high-quality resident care
