Position Overview
Make a meaningful impact every day as a Center Well Home Health Nurse. You will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you will manage care plans that support recovery and help patients get back to the life they love. You will report to the Clinical Manager.

Responsibilities- Provide skilled nursing interventions in the treatment of the patient’s illness, rehabilitative needs and preventative care, under the direction of a Registered Nurse.
- Confer with the RN Supervisor regarding needed changes in the Plan of Treatment.
- Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team regarding patient/family needs and status changes.
- Participate in care coordination activities and discharge planning.
- Prepare appropriate medical documentation and visit or shift reports, updates and summaries of patient records, and confer with other health care disciplines in providing optimum patient care.
- Use your skills to make an impact.
- Licensed Practical Nurse, licensed in the state of North Carolina, without restrictions.
- A valid driver’s license, auto insurance, and reliable transportation.
- A valid CPR certification.
- Home Health experience.
Pay Range: $33.00 – $47.00 per visit/unit. Base pay: $53,000 – $73,000 per year. 40 hours per week.
Driving StatementYou will be a part of Humana’s driver safety program and therefore will need a valid state driver’s license and proof of personal vehicle liability insurance with at least 100/300/100 limits.
TB StatementBecause you will be patient‑facing, you will be enrolled in Humana’s Tuberculosis (TB) screening program. If selected for this role, we will require you to undergo TB screening.
