The Registered Nurse collects and analyses patient data, formulates an individualized plan of care, determines progress toward the attainment of outcomes, coordinates care delivery, promotes a safe practice environment, and advocates mutual respect with patients, families, and other healthcare providers.

• Conduct in‑home assessments, and develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
• Communicate significant clinical changes or findings to the physicians involved in the plan of care and other health care practitioners.
• Educate patients and families on disease management, medication administration, and preventative care.
• Monitor vital signs and observe patients for any changes in health condition and provide wound care.
• Collaborate with other healthcare professionals to ensure holistic care for patients.
• Maintain accurate and up‑to‑date medical records and documentation promptly.
• Supervise and instruct home health aides. Accept student nurses as directed.
• Takes on‑call during evening hours, nights, holidays, and weekends on a rotating basis.

Associate Degree in Nursing (ADN) required. Bachelor Degree Nursing (BSN) preferred.

Two years’ experience preferred.

Valid Kentucky Board of Nursing (KBN) license required. Valid Kentucky driver’s license required.
