The Registered NurseThe Registered Nurse collects and analyses patient data, formulates an individualized plan of care, determines progress toward the attainment of outcomes, coordinates care delivery, promotes a safe practice environment, and advocates mutual respect with patients, families, and other healthcare providers.
Duties/Responsibilities- Conduct in‑home assessments, and develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
- Communicate significant clinical changes or findings to the physicians involved in the plan of care and other health care practitioners.
- Educate patients and families on disease management, medication administration, and preventative care.
- Monitor vital signs and observe patients for any changes in health condition and provide wound care.
- Collaborate with other healthcare professionals to ensure holistic care for patients.
- Maintain accurate and up‑to‑date medical records and documentation promptly.
- Supervise and instruct home health aides. Accept student nurses as directed.
- Takes on‑call during evening hours, nights, holidays, and weekends on a rotating basis.
Associate Degree in Nursing (ADN) required. Bachelor Degree Nursing (BSN) preferred.
ExperienceTwo years’ experience preferred.
Licensure/CertificationValid Kentucky Board of Nursing (KBN) license required. Valid Kentucky driver’s license required.

The employee is regularly required to stand, walk, and be on foot for long periods. Must be able to tolerate PPE for long periods.
