Job TypeFull-Time
Educational Requirements- Bachelor’s degree in Human Services or a related field required
- Utilize evidence-based models including Wraparound, Person-Centered Planning, Family Psychoeducation, Coordinated Specialty Care (CSC), and Care Coordination to support clients and families
- Facilitate team development and guide the creation of collaborative, comprehensive care plans
- Assist clients and families in identifying goals, strengths, needs, and barriers to treatment
- Link individuals and families with community resources, supports, and services as appropriate
- Facilitate or co-facilitate quarterly team meetings to ensure continuity and coordination of care
- Collaborate with interdisciplinary teams, providers, schools, and community partners to support positive client outcomes
- Assist the Program Director with outreach efforts, community education, and program fidelity measures
- Maintain accurate documentation and records in accordance with agency policies and applicable regulations
- Participate in required trainings, meetings, and supervision
- Travel within the service area as needed to meet with clients, families, and community partners
- Provide services and coverage across all ten (10) counties within the FIVCO and Gateway regions
- Perform all other duties as assigned
- Minimum of one (1) year of post-graduate paid experience working directly with the target population
- Strong understanding of recovery-oriented and family-centered service approaches
- Ability to work effectively with diverse populations and identify individualized needs and supports
- Knowledge of evidence-based practices and care coordination principles preferred
- Strong communication, organization, teamwork, and collaboration skills
- Ability to build positive working relationships with consumers, families, and community agencies
- Flexibility and adaptability in a fast-paced work environment
- Completion of required trainings including SED, SMI, iHOPE CSC, and additional trainings as directed by the agency required within designated timeframe
