Registered Nurse II​/III MC​/CHC Per Diem - MSC Youth - Enhanced Care Management

Job Summary

The Riverside University Health System (RUHS) Community Health Centers is recruiting an experienced Per Diem Registered Nurse II/III to serve the MSC Pediatrics Clinic – Youth Enhanced Care Management department in Moreno Valley. The primary location is the MSC General Pediatrics Clinic; the position requires travel to other Community Health Center sites to support member care coordination and care management services.

• Manage a caseload of members with complex medical and behavioral health needs, providing care coordination, chronic disease education, medication reconciliation, and clinical oversight.
• Collaborate with members, families, primary care providers, pharmacies, hospitals, and interdisciplinary teams to develop individualized health action plans and promote healthy lifestyle changes.
• Coordinate transitions of care, ensuring timely completion of required assessments and screenings.
• Conduct in‑person and telephonic member outreach, track health outcomes, address clinical concerns and medication alerts, and support member engagement through evidence‑based communication strategies, including motivational interviewing.
• Participate in quality assurance reviews and act as a team leader or relief charge nurse when required.

• Bachelor’s degree in nursing or a related health‑care field, and a current valid California Registered Nurse license.
• Minimum one year of full‑time experience as a Registered Nurse in an acute care, outpatient clinic, or community health agency setting (or equivalent). Three years of experience may be substituted for a Bachelor’s degree; a Master’s degree may substitute for two years.
• Experience in pediatric care and case management preferred.
• Bilingual in Spanish is a competitive advantage.

• Basic Life Support (BLS) certification issued by the American Heart Association.
• Valid California Driver’s License.

• Maintain a current valid California Registered Nurse license.

5/40, Monday to Friday – 8:00am to 4:30pm

Registered Nurse II – $70.81 per hour;
Registered Nurse III – $80.25 per hour
