Position Summary

NP/PA Physical Medicine & Rehabilitation (PM&R) Clinician at Comprehensive Rehab Consultants (CRC). The role involves providing PM&R services exclusively at a designated skilled nursing facility (SNF) or post‑acute rehab center in Morgan Hill, CA.

• Provide specialized PM&R/Physiatry services as a rehab consultant.
• Represent CRC through clinical leadership and collaboration with interdisciplinary teams.
• Collaborate with PCP teams on management of common SNF conditions (CVA, progressive neurological conditions, postoperative orthopedic surgeries).
• Work with PT, OT, and SLP teams to improve patient function and quality of life.
• Follow therapy patients to track progress.
• Assess and collaborate on management of barriers to therapy including joint or neuropathic pain, spasticity, neurogenic bowel/bladder, etc.
• Participate in multidisciplinary meetings, bridging the gap between therapy and internal medicine.
• Engage in ongoing education and culture‑building opportunities.

• Nurse Practitioner/APRN or Physician Assistant license;
  California license.
• Licensed to work in the U.S.; must be able to commute to Morgan Hill, CA.
• Experience in post‑acute care preferred.
• Strong interpersonal and collaborative skills, empathy, and a passion for geriatric post‑acute healthcare.
• Ability to function autonomously, embrace change, and commit to continuous improvement.

• Part‑time (16hrs/week): $85/hr.
• Full‑time (24hrs/week): $75,000—$95,000 annually, plus bonus.
• Schedule:
  
  8‑hour shifts, flexible around 9AM–4PM, 2–3 days per week, Monday‑Friday.
