Are you a compassionate Licensed Practical Nurse (LPN) looking for an opportunity to positively impact the lives of adults in a long-term care environment? We are seeking a dedicated nurse to join our team in Morgan Hill, California, providing exceptional care and support to individuals with intellectual and developmental disabilities (IDD) in a structured residential setting.
In this role, you'll combine your clinical expertise with behavioral support and resident-centered care to help create a safe, therapeutic, and supportive environment.
What You'll DoAs an LPN, you will:
- Deliver quality nursing care within your scope of practice
- Provide general psychiatric and long-term care nursing services
- Support and implement individualized treatment and behavior plans
- Monitor resident health status and communicate concerns appropriately
- Participate in interdisciplinary care planning meetings
- Supervise DSPs and CNAs when functioning as lead staff
- Provide direct resident care and therapeutic support throughout assigned shifts
- Maintain accurate documentation using paper charting systems
- Follow facility protocols and mandated reporter guidelines
- Coordinate patient care and contribute to positive resident outcomes
- Shift: NOC (Night Shift)

40 hours per week - Contract Duration: 13 weeks

- Current Licensed Practical Nurse (LPN) License in the state of practice
- Current BLS Certification
- Current Health Certificate (if applicable)
- Current PPD or Chest X-Ray
- Successful completion of background screening
- Experience working with adults with Intellectual and Developmental Disabilities (IDD)
- Successful completion of clinical specialty screening assessments with a score of 80% or greater
- One year of professional nursing experience
- Long-term care, behavioral health, psychiatric, or assisted living experience

The Long Term Care LPN assumes responsibility and accountability for the delivery of care for patients in a Long Term Care/Assisted Living Facility. The nurse demonstrates ability to make clinical judgments in an effective and efficient manner with supervision, as well as critical thinking and performance ability in the coordination of patient care. Additionally, this individual performs according to nursing standards and is accountable in managing patient care and assisting others in the management of patient care.
Minimum Requirements:
- Current Licensed Practical Nurse License for the state in which the nurse practices
- Current Health Certificate (if applicable)
- Current PPD or Chest X-Ray
- One year prior professional nursing experience preferred
- Current BLS card
- Successful completion of screening tests for the clinical specialty area(s) in which the LPN practices with a score of 80% or greater
- Successful completion of Background Screening Process
