We’re looking for a Care Navigator (Chronic Care Management Specialist) to help us revolutionize patient care in Morganton, NC.
PositionChronic Care Management Specialist (Care Navigator)
LocationMorganton, NC (on‑site position at our outpatient care clinic)
ScheduleMonday‑Friday with occasional weekends to close out month‑end processes.

- Educate and motivate patients to achieve their health goals
- Be the friendly voice and listening ear that patients look forward to hearing from
- Certified Medical Assistants (MAs) passionate about patient care
- Empathetic individuals with excellent communication skills
- Tech‑savvy professionals comfortable with digital health platforms
- Detail‑oriented team players able to juggle multiple priorities
- Healthcare enthusiasts eager to learn and grow in an innovative environment
- Nationally recognized Medical Assistant (MA) certification
- LPN License
