LPN – Outpatient Clinic PACE

We are hiring a Licensed Practical Nurse (LPN) for our Outpatient Clinic PACE (Program of All Inclusive Care for the Elderly) with Care Partners Health Services. The position is Monday–Friday, closed on weekends.

Job Summary

The LPN plays a vital role in supporting participants through comprehensive medication oversight and team coordination. Guided by the PACE model of care, the LPN ensures medications are administered and documented accurately, assesses changes in condition, and provides hands‑on care in collaboration with providers, pharmacy, caregivers, and the interdisciplinary team.

Essential Accountabilities

Provides direct patient care in an outpatient setting, evaluates outcomes, and adjusts nursing care processes to adhere to legal, ethical, and clinical standards.

Performance Criteria

Evaluates and records patient vitals.

Assists provider with therapeutic and diagnostic procedures.

Administers oral/injectable medications as ordered and monitors patient’s response.

Delegates to other licensed and unlicensed staff according to scope of practice and competency.

Delivers quality customer service to all patients and family members.

Triages participants and addresses their needs and concerns, making clinical judgement to determine necessity of physician involvement.

Coordinates care with specialist providers and nursing facilities.

Coordinating with care team to implement goals and interventions as it relates to the clinical team.

Assesses participants’ medical needs and creates a plan of care to support community success.

Acts as coordinator of care, orders, and interventions as prescribed with SNF Partners.

Collaborates and consults with patients, colleagues, and interdisciplinary team to achieve desired outcomes.

Additional Performance Criteria

Regularly documents and reports data, patient response, and progress.
Individualizes patient teaching and care plan based on patient’s needs.
Advocates for patients and families to achieve desired outcomes.
Identifies problems and implements measures to meet needs of patients and families throughout continuum of care.

Qualifications

Basic Cardiac Life Support certification within 30 days of employment start date (LPN/LVN).
Licensed Practical or Vocational Nurse certification within 2 years of employment start date.
Associate Degree.
