LPN – Outpatient Clinic PACE
We are hiring a Licensed Practical Nurse (LPN) for our Outpatient Clinic PACE (Program of All Inclusive Care for the Elderly) with Care Partners Health Services. The position is Monday–Friday, closed on weekends.
Job SummaryThe LPN plays a vital role in supporting participants through comprehensive medication oversight and team coordination. Guided by the PACE model of care, the LPN ensures medications are administered and documented accurately, assesses changes in condition, and provides hands‑on care in collaboration with providers, pharmacy, caregivers, and the interdisciplinary team.
Essential Accountabilities- Provides direct patient care in an outpatient setting, evaluates outcomes, and adjusts nursing care processes to adhere to legal, ethical, and clinical standards.
- Regularly documents and reports data, patient response, and progress.
- Individualizes patient teaching and care plan based on patient’s needs.
- Advocates for patients and families to achieve desired outcomes.
- Identifies problems and implements measures to meet needs of patients and families throughout continuum of care.
- Basic Cardiac Life Support certification within 30 days of employment start date (LPN/LVN).
- Licensed Practical or Vocational Nurse certification within 2 years of employment start date.
- Associate Degree.
