Care Partners seeks a compassionate Registered Nurse Homecare Hospice professional to deliver high-quality, in-home end-of-life care across our Connecticut community. In this role, you will perform comprehensive patient assessments, manage pain and symptoms, and coordinate individualized care plans with our network of partnered providers. You will educate patients and families, support them through the end-of-life journey, and ensure accurate EHR documentation.
The ideal candidate brings strong clinical judgment, hospice or homecare experience, and a commitment to patient-centered, community-focused care aligned with Care Partners’ mission.
