Licensed Practical Nurse – Home Health

As an LPN/LVN with our Home Health Team, you will deliver compassionate, personalized care to patients in the comfort of their homes. In collaboration with registered nurses and the care team, you’ll manage medical needs and promote independence, caring for patients like family, one home visit at a time.

• Provide nursing care in the home, including wound care, medication administration, and support during procedures.
• Document assessments, interventions, and outcomes clearly and thoroughly to support safe, continuous care.
• Monitor patient status and alert the care team of any changes to ensure timely, coordinated responses.
• Educate patients and families to support medication adherence, strengthen understanding of health conditions, and promote greater independence at home.
• Collaborate closely with RNs, providers, therapists, and care coordinators to deliver high‑quality, personalized care in the home setting.

• Licensed Practical Nurse or Licensed Vocational Nurse in good standing with the state board.
• Associate Degree in Nursing or completion of a vocational nursing program.
• Experience in home health or related patient care settings is preferred.
• Excellent communication skills and ability to work independently and as part of a multidisciplinary team.
