Position: LICENSED PRACTICAL NURSE

Transylvania Regional Hospital, part of Mission Health, is seeking a Licensed Practical Nurse in our Healthcare & Medical Services department. In this acute care role, you will provide direct patient care, administer medications, support care planning, and collaborate with RNs and physicians to ensure safe, high-quality treatment. You'll work in a fast-paced environment, monitoring patient status and documenting care accurately. Responsibilities

  • Provide direct bedside nursing care to acute care patients within the LPN scope of practice under the supervision of an RN or provider
  • Obtain, record, and monitor vital signs, intake and output, and patient observations, promptly reporting changes in condition
  • Administer medications and treatments as ordered, monitoring and documenting patient responses and side effects
  • Assist with patient admissions, transfers, and discharges, including gathering histories, completing documentation, and reinforcing discharge instructions
  • Perform basic clinical procedures such as wound care, specimen collection, hygiene and comfort measures, and assistance with activities of daily living
  • Support patient and family education on diagnoses, medications, and home care needs in coordination with the RN and care team
  • Maintain accurate, timely documentation in the electronic medical record, ensuring compliance with hospital and regulatory standards
  • Collaborate with RNs, physicians, and interdisciplinary team members to support care plans and coordinate services
  • Participate in patient safety initiatives including fall prevention, infection control measures, and safe medication practices
  • Contribute to a culture of teamwork, respect, and continuous improvement, including attending required trainings and skills development sessions
  • Assist in emergency situations within scope of practice, supporting the care team in rapid response or code events as directed
  • Maintain a clean, safe, and organized patient care environment, ensuring proper handling of equipment and supplies
