- Administer and monitor psychotropic medications as prescribed
- Partner with the team psychiatrist to update medication protocols and care plans
- Participate in daily in-person ACT Team meetings to ensure coordinated, person‑centered treatment
- Provide crisis assessment and intervention as part of a rotating on‑call schedule
- Deliver nursing care in clients’ homes and community settings
- Educate individuals on health management, wellness, and medication adherence
- Advocate for client choice and connect individuals with needed community resources
- Strong communication, documentation, and relationship‑building abilities
