Performs patient assessment and plans, delivers and evaluates patient education to selected populations across the continuums of illness and lifespan. Assumes a role as a resource person by participating in community outreach and education support.

* MINIMUM QUALIFICATIONS:

**** EDUCATION, CERTIFICATION, AND/OR LICENSURE:
** 1. Current unencumbered licensure with the WV Board of Registered Professional Nurses, or appropriate state board where services will be provided, as a registered professional nurse OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).2. Obtain Certification in area of specialty within 1,000 hours of documented diabetes education teaching.
3. Obtain certification in Basic Life Support within 30 days of hire date.

* EXPERIENCE:

** 1. Two (2) years of clinical experience with One (1) year in the care of diabetic patients.

DUTIES AND RESPONSIBILITIES:

1. Identifies education needs of the community and those with diabetes.
2. Attends diabetes advisory committee meetings as assigned.
3. Performs patient assessment, delivers education and evaluates patient’s obtainment of goals.
4. Assists in maintaining the American Diabetes Association certification as applicable.
5. Participates in community outreach and educational support.
6. Advocates on behalf of the patient and families.
7. Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes.
8. Participates in performance improvement activities to assure quality patient care.
10. Completes daily rounds to assess patient status.
11. Ensures all necessary testing and scheduling is complete prior to necessary procedures.
12. Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.
13. Leads the planning, implementation and evaluation of the Outpatient Diabetes and Nutrition Education Program.
