This position functions as an integral member of Valley's Assertive Community Treatment (V-ACT) Program. This individual works to serve severe and persistently mentally ill consumers within Valley's catchment area. Assists with the appropriateness of new referrals to the V-ACT Program, as necessary and serves as the RN performing medical oversight of the consumers' needs. The RN acts as a medical resource individual for the team.
The RN will provide direction and oversight to program LPN(s), as needed and directed. The RN will uphold the agency and program mission/values by offering professional nursing services to clients to ensure optimum service planning and delivery.
ESSENTIAL DUTIES:
* Provides direct nursing services to program clients, including medication delivery/administration, blood draws, and any other needed nursing/medical assessment within RN scope of practice.
* Provides medical oversight to clients as necessary.
* Provides transportation to program clients as needed.
* Attends psychiatric and medical appointments with program clients as necessary.
* Monitors and organizes medical sections of client charts.
* Ensure client Medication Lists and MAR's are current and accurate at all times.
* Answers medication questions and provides medical education for staff and clients.
* Administers all medications as ordered by a physician, and assists with medication refills.
* Follows approved standard nursing practices and guidelines when administering all medications to clients. This will include proper documentation and timeliness of the administration of medications as well as observation of the client.
* Participate in treatment planning activities to ensure that ongoing medical services are appropriate and are provided in coordination with other program services.
* Oversees care provider in the delivery of medical services to clients.
* Complies with and implements all physicians' orders.
* Helps to develop, reviews and ensures implementation of care plans in compliance with Valley's, State, Federal guidelines and physician's orders.
* Completes Personal Care evaluations and monitors clients receiving Personal Care services.
* Ensures the timeliness and accuracy of information documented into the clients records.
* Participate as a member of the professional interdisciplinary treatment team as client needs dictate.
* Documents all client contacts and services either collaboratively or within 24 hours of the service being provided.
* Support and help in the training of new employees as needed.
* Adhere to professional code of ethics in performing nursing services.
* Conducts trainings as assigned.
* Adheres to Valley's Direct Client Service Time, clinical quality, and compliance standards.
* Willing to drive personal and/or agency vehicle in less-than-ideal weather conditions to ensure client needs are met.
* Provide direction and oversight to program LPN(s), as needed and directed by the Program Manager.
* OTHER DUTIES AS ASSIGNED.
