The LPN provides quality nursing care and services to residents in a respectful and dignified manner, ensuring the delivery of care and services. The LPN coordinates physical, social, and emotional aspects of assisted living care and services in accordance with the physician’s orders and the resident’s choices.
Responsibilities- Execution of the medication management plan, including periodic review of documentation, observation of plan effectiveness, and communication to the physician when changes are needed or outcomes are not appropriate.
- Continuous assessment of resident needs with updates to appropriate individuals regarding care, services, and changes in condition.
- Implement measures to ensure resident wellness and safety and document those measures appropriately.
- Work in a team fashion to meet resident needs.
- Respond to emergencies in a timely, calm, and competent manner.
- Report incidents and changes in resident condition to the Healthcare Director with proper documentation.
- Be an active part of all activities, including community events.
- Assist with the delivery of meals in the dining room, monitor resident acceptance of diets, and assure adequate nutrition.
- Maintain high standards in record keeping, resident, and staff documentation.
- Establish and maintain a positive working relationship with all departments.
- Identify risk factors for residents and initiate interventions with appropriate documentation.
- Provide a thorough report of resident status to the incoming nursing team at each shift change and in the Daily Log.
- Provide motivation and direction to CNAs/RMAs.
- Must be certified or willing to take and pass the certification classes required by the state in which employed.
- Attend required trainings, CEU’s, and staff meetings.
- Adhere to and implement all policies and procedures.
- Practice proper body mechanics and safe resident transferring techniques.
- Adhere to attendance policies; report to work on time and prepared to begin duties.
- Complete all other assigned duties.
- Current LPN license to practice nursing in the state where the community is located, held in good standing with the Board of Nursing.
- Experience working with the senior population, preferably in assisted living or LTC.
- Team player who works well with others and has experience supervising staff.
- Must possess critical thinking skills and the ability to communicate verbally and in writing effectively.
- Demonstrated ability to manage time efficiently with strong organizational skills.
- Physically able to sit, stand, bend, kneel, walk, and lift up to 50 lbs.
- Willing to work an 8‑16 hour flexible schedule, including weekends and holidays.
- Must attend all required trainings, CEU’s, and staff meetings.
- Must be certified or willing to take and pass the certification class required by the state in which employed.
