Role:
Licensed Practical Nurse (LPN) in home health care, providing patient-centered, 1:1 care in clients' homes.
Responsibilities- Administer medications, feedings, oxygen, and ostomy care according to care plans.
- Accurately document nursing actions, patient progress, and communicate with families, case managers, and physicians.
- Coordinate supplies, medications, and ensure patient needs are met following medical instructions.
- Maintain client dignity and independence while providing personalized care.
- Licensed in the state of practice with an active, unrestricted registration.
- Valid driver’s license, proof of insurance, and acceptable driving record.
- CPR certification.
- Preferably 1+ years of nursing experience, with home health experience advantageous.
