Under general supervision, The Licensed Practical Nurse provides basic nursing care to clients in public health settings including clinics, home visits, and community outreach programs. Responsibilities include monitoring vital signs, administering medications, assiting with procedures, documenting patient conditions, and supporting health education efforts. May assist in programs related to chronic disease management, maternal and child health, immunizations, and communicable disease control.
This position does not include supervisory responsibilities. Employee has a good understanding of the tasks and associated processes but still requires periodic guidance to continue the learning process. Employee has learned the basics of the process but still requires periodic feedback in order to continue knowledge and professional growth.
Job Duties
- Provide direct patient care, including taking vital signs, performing assessments, and documenting findings.
- administer medications and immunizations as ordered by authorized providers.
- Assist with clinical procedures such as wound care, specimen collection, and screenings.
- Monitor patients for adverse reactions and report change in condition promptly.
- Maintain accurate and timely documentation in electronic health records (EHR).
- Educate patients on medication use, disease prevention, and health maintenance.
- Ensure compliance with infection control and safety protocols.
- Prepare and maintain exam rooms, instruments, and supplies for patient visits.
- Support public health initiatives such as vaccination clinics and community outreach events.
- Participate in emergency preparedness and response activities as assigned.
- Comply with all safety policies, practices and procedures. Report all unsafe activities to supervisor and/or Human Resources.
- Participate in proactive team efforts to achieve departmental and company goals.
- Perform other duties as assigned by supervisor or above.
- Provide leadership to others through example and sharing of knowledge/skill.
This position is considered having moderate physical activity performing some what strenuous daily activities of a primarily product/technical nature.
Physical DemandsThe employee must be able to stand and walk for extended periods during patient care and outreach events. Tasks involve bending, stooping, and reaching to assist patients and handle equipment. Manual jdexterity is essential for administering injections and operating medical instruments. The role may require lifting or moving patients and equipment weighing up to 50 pounds with assistance. Visual acuity is needed for reading charts, medication labels, and performing clinical tasks.
Hearing ability is required for patient communication and monitoring alarms. With or without reasonable accommodation, the employee must be able to perform these essential functions.
The employee works in a clinical setting within the health department, including exam rooms, vaccination clinics, and outreach sites. The environment is climate-controlled but may involve exposure to infectious diseases, biohazard materials, and cleaning chemicals. PPE such as gloves, masks and gowns is required for certain tasks. The role involves frequent interaction with patients, staff, and community members. Occasional travel to off-site clinics or community events may be necessary.

Completion of an accredited practical nursing program; current LPN license in the state of West Virginia. One year of clinical experience in a public health or outpatient setting is preferred. Proficiency with EHR systems, scheduling software, and basic office applications. Familiarity with medical devices and diagnostic equipment.
