Key Responsibilities:
Medication Administration:
Administer prescribed medications to residents in accordance with physician orders and facility policies.
Follow the five rights of medication administration: right patient, right drug, right dose, right route, and right time.
Monitor residents for any side effects or adverse reactions and report concerns to the supervising nurse or physician.
Ensure proper documentation of medication administration in the electronic health record (EHR) system.
Resident Care and Support:
Provide compassionate care and assistance to residents, addressing their personal needs and concerns.
Assist residents with activities of daily living (ADLs) when necessary, such as grooming, feeding, and mobility.
Ensure that residents are comfortable and monitor their overall health condition, communicating any changes to the nursing staff.
Compliance and Safety:
Ensure that medication storage, handling, and administration comply with all state and facility regulations.
Follow infection control protocols and safety procedures to ensure a clean and safe environment for residents.
Participate in audits, inspections, and medication reconciliation processes to ensure accuracy and compliance with facility policies.
Collaboration and Communication:
Work closely with RNs, LPNs, and Certified Nursing Assistants (CNAs) to ensure coordinated and holistic care for residents.
Communicate effectively with residents and their families, addressing questions and providing updates on care.
Respond to emergency situations promptly and assist nursing staff as needed.
Documentation and Reporting:
Accurately document all medication administration and any changes in residents' conditions in the EHR system.
Report any medication errors or discrepancies immediately to the supervising nurse and follow facility procedures for incident reporting.
Participate in care planning meetings as required, providing input on residents' medication needs and overall health status.
Education:
High school diploma or equivalent required.
Certification:
Medication Technician certification. Certification in CPR and First Aid preferred.
Experience:
Previous experience in a healthcare, long-term care, or assisted living setting is preferred but not required.
Skills:
Strong attention to detail and accuracy in administering medications.
Excellent communication and interpersonal skills, with a compassionate and resident-centered approach to care.
Ability to work effectively as part of a team and follow directions from nursing staff.
Basic understanding of medical terminology, medication types, and potential side effects.#LIONSTONE
