Integrity House operates 16 licensed treatment programs across New Jersey as well as community outreach and prevention programs, and serves over 3,000 individuals every year.

We are excited to announce our Case Manager opening in Morris Plains, NJ and Secaucus, NJ. The Case Manager is responsible for coordinating participant enrollment, assessments, Individual Employment Plans (IEPs), supportive services, workforce development coordination, and employment retention services for the Pathways to Recovery program. The Case Manager serves as the primary point of contact for participants and collaborates with employers, One-Stop Career Centers, training providers, and community partners to remove barriers to employment and promote long-term success.

Key Responsibilities Participant Engagement, Assessment & Service Planning
  • Recruit, screen, and enroll eligible participants through community outreach, referrals, workforce agencies, treatment providers, criminal justice agencies, healthcare organizations, and other community partners.
  • Conduct participant orientation, explaining program expectations, participant rights and responsibilities, available services, confidentiality, attendance requirements, and employment expectations.
  • Complete comprehensive participant assessments to evaluate employment history, educational background, vocational interests, work readiness, strengths, barriers, supportive service needs, and career goals.
  • Coordinate required assessments including workforce readiness evaluations, literacy assessments, vocational interest inventories, and other assessment tools utilized by the program.
  • Develop Individual Employment Plans (IEPs) that establish measurable employment, education, training, and supportive service goals.
  • Review and revise IEPs regularly based upon participant progress, changing needs, and employment outcomes.
  • Maintain regular participant contact to encourage engagement, accountability, and successful completion of program objectives.
Case Management & Supportive Services
  • Maintain an assigned caseload and provide ongoing individualized case management services throughout participant enrollment.
  • Conduct regularly scheduled case management sessions to assess progress, identify barriers, and develop appropriate intervention strategies.
  • Coordinate supportive services necessary to reduce barriers to employment including transportation, childcare, work clothing, identification documents, housing referrals, legal assistance, work supplies, and emergency resources.
  • Coordinate referrals to behavioral health providers, substance use treatment programs, healthcare providers, educational institutions, financial assistance programs, and community-based organizations.
  • Utilize motivational interviewing, trauma-informed care, and strengths-based interventions to encourage participant engagement and promote self-sufficiency.
  • Monitor participant participation in scheduled services and address attendance concerns through individualized intervention planning.
  • Maintain collaborative communication with internal staff and external providers to ensure continuity of care and coordinated service delivery.
Workforce Development & Employment Coordination
  • Coordinate participant enrollment into workforce readiness programming, occupational training, educational services, and career development activities.
  • Collaborate with the Workforce Readiness Instructor, GED Instructor, and Employment Specialist to monitor participant progress and ensure successful completion of assigned activities.
  • Assist participants in identifying career pathways consistent with their interests, skills, abilities, and long-term employment goals.
  • Prepare participants for employment through career planning, interview preparation, workplace readiness reinforcement, and coordination of occupational training opportunities.
  • Monitor participant completion of certifications, credentials, and training requirements necessary for employment.
  • Coordinate participant referrals for interviews, job placement opportunities, internships, and work-based learning experiences.
  • Conduct employment retention follow-up, identify workplace concerns, and coordinate interventions to promote long-term employment success.
Qualifications
