• Evaluate and treat patients with functional, cognitive, and physical impairments
• Address ADLs, IADLs, upper‑extremity rehab, and adaptive strategies
• Provide 1:1 patient‑centered care
• Collaborate with PT, SLP, nursing, and case management
• Complete accurate documentation and discharge planning

• Master’s or Doctorate Degree in Occupational Therapy
• Active (or pending) state OT license
• SNF experience preferred, not required
