Physical Therapist

We are seeking a licensed Physical Therapist to provide personalized, in-home and senior living care, focusing on improving mobility, independence, and quality of life for older adults.

• Offering one-on-one therapy sessions in private residences and senior communities
• Performing evaluations, developing individualized treatment plans, and delivering evidence-based care
• Supporting patient wellness, mobility, and functional independence through therapy
• Documenting care in electronic medical records in compliance with standards
• Collaborating with an interdisciplinary team of healthcare professionals
• Educating patients, caregivers, and staff to promote safety and aging in place
• Utilizing telehealth and virtual care to enhance access and flexibility

• Valid PT license or eligibility in practice state
• Degree from an accredited Physical Therapy program
• Proficiency in basic computer and EMR use
• Current CPR certification

Physical demands involve: exerting moderate effort, assisting with patient transfers, repositioning, walking, climbing stairs, and driving to care locations, with appropriate PPE use.
