Position Overview:

As a Licensed Clinical Social Worker (LCSW), you will provide behavioral health services to patients in post-acute and skilled nursing facilities. Your responsibilities will include conducting mental health evaluations, developing individualized treatment plans, providing psychotherapy, and collaborating with interdisciplinary care teams to support patients' emotional and psychological well-being.

What You’ll Do:

• Conduct comprehensive behavioral health consultations and initial mental health evaluations.
• Develop individualized, evidence-based treatment plans.
• Assess patients using standardized behavioral health screening and assessment tools.
• Provide brief, solution-focused therapy and psychotherapy for adult patients.
• Perform crisis assessments, triage, and implement appropriate interventions.
• Coordinate care with physicians, nursing staff, and other healthcare professionals.
• Connect patients and families with appropriate community resources and referral services.
• Promote wellness through education and behavioral health interventions.
• Maintain accurate documentation and scheduling within the electronic health record (EHR).
• Participate in interdisciplinary care planning and perform other duties as assigned.

Who Should Apply?

Master's Degree in Social Work.

Active Licensed Clinical Social Worker (LCSW) license.

Minimum of two (2) years of clinical experience providing behavioral health services.

Compassionate, patient-centered approach to care.

Strong clinical assessment and critical thinking skills.

Excellent communication and interpersonal abilities.

Ability to work independently while collaborating with interdisciplinary teams.

Strong crisis intervention and problem-solving skills.

Comfortable using electronic health record (EHR) systems.
