- Assessment and Evaluation:
Conduct thorough assessments of residents' speech, language, cognitive, and swallowing abilities. - Individualized Care Plans:
Develop customized intervention plans to address communication and swallowing difficulties. - Section GG Data Collection:
Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, and Significant Change assessments, Section K, BIMS, PhQ2-9, and other clinical data as applicable. - Treatment:
Administer speech therapy interventions, including exercises, communication strategies, and swallowing therapy. - Documentation:
Maintain accurate and timely documentation of resident progress and any interventions provided. Comfortable and knowledgeable with Point of Service( PoS) care delivery and documentation. Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, HMO/Manage care progress updates in EMRs( Net Health/PCC). - Education:
Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress. Conducts remote check‑in through remote therapeutic monitoring(RTM) with discharged patients to home or next level of care. - Interdisciplinary
Collaboration:
Collaborate with other rehab staff, such as Physical Therapists and Occupational Therapists, to provide holistic care for residents.
