JOB DESCRIPTION

Atlantic Health is seeking a Diabetes Nurse Practitioner (NP) in Pompton Plains, New Jersey. The NP will support both outpatient services in Pompton Plains, as well as inpatient services at Chilton Medical Center.

As a Diabetes NP, you will have a crucial role in delivering specialized diabetes care to adult patients aged 18 and above. Your responsibilities will include providing comprehensive diabetes care, ensuring high-quality evidence-based practice, collaborating with a multidisciplinary team, developing individualized treatment plans, educating and supporting patients in self-management, contributing to the growth of the Endocrine program through quality improvement initiatives, and engaging in continuous learning and professional development.

• Graduated from an accredited NP program
• Master's degree required
• Certified Diabetes Counselor Education Specialist (CDCES) or ability to obtain a certification within 1 year of hire
• ACLS and BLS certification required
• 2-3 years of clinical experience preferred, in an acute care or physician practice setting managing patients with diabetes; new graduates will be considered
• Excellent communication and computer skills
• Strong work ethic and a team player who can work independently
