GIP Hospice - Per Diem, Weekends, Northern​/NJ

Position Summary

Per diem Hospice Registered Nurse covering Monday through Friday, including weekends and on‑call as needed. Works with adult and geriatric patients across Morristown Medical Center and Overlook Medical Center.

Compensation starts at $43.00 per hour and increases with experience. Mileage reimbursement is provided.

Reports to Assistant Manager.

• Performs patient assessments and determines appropriateness for hospice care.
• Develops, implements, and evaluates patient and family plans of care.
• Maintains visit productivity standards and demonstrates knowledge of insurance reimbursement.
• Completes discharge assessments, including laptop transfer of data twice daily and admission assessments within 24 hours.
• Provides age‑appropriate teaching to patients and families and documents education tools used.
• Manages case‑management responsibilities, including revising care plans, prioritizing needs, and maintaining compliance with hospice regulations and reimbursement guidelines.
• Collaborates with interdisciplinary teams (IDT), attends 2‑week IDT meetings, and documents progress toward goals.
• Ensures a safe environment, follows infection control measures, demonstrates pharmacology knowledge, reconciles medications, and maintains medication orders.
• Participates in monthly staff meetings, PI meetings, and other committees as requested.
• Engages in performance improvement activities, reviews health records, attends UR meetings, and mentors new employees.
• Supports agency growth by providing orientation, accepting additional assignments, and cross‑training.

• Current New Jersey RN License.
• Good standing New Jersey driver’s license.
• Associate’s degree in Nursing.
• Basic Life Support (BLS) certification.

• Minimum of one year experience in acute, subacute, or hospice/home care setting.
• Experience using an Electronic Medical Record (EMR).
• BSN degree.

Starting at $43.00 per hour with potential increases based on experience. Mileage reimbursement included.
