Key Responsibilities:

• Provide high-quality patient care to our residents, including administering medications, wound care, and vital sign monitoring

• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans

• Monitor and document patient progress, reporting any changes or concerns to the healthcare team

• Assist with daily living activities, such as bathing, dressing, and grooming

• Utilize strong communication skills to maintain open lines of communication with patients, families, and healthcare teams
