American Traveler is hiring an experienced RN Case Manager with acute care and sub-acute patient experience for an in-patient case management role requiring an active NJ or compact RN license.

• Hospital-based Care Management unit position
• Day shift schedule from 8:00 AM to 4:00 PM
• 37.5 hours per week across 5 days
• Rotating weekend coverage required approximately every 4th or 5th weekend
• Holiday coverage required for one summer and one winter holiday
• Floating to a nearby sister hospital may be required as needed
• No on-call responsibilities
• Uses Epic EMR charting system

• Active RN license (NJ or Compact) required
• BSN required
• CCMC certification preferred
• 2 years of acute care experience strongly preferred
• In-patient case management experience required
• Experience with sub-acute patients required
• Background in Managed Care and Case Management

• Coordinate patients through the acute care hospitalization process
• Manage all aspects of discharge planning and transition of care
• Perform utilization management and quality screenings
• Must reside outside a 50-mile radius of the facility to be eligible for consideration
