We're seeking a compassionate Per Diem Hospice Registered Nurse to join our exceptional hospice team. In this role, you'll provide expert nursing care to adult and geriatric hospice patients at Morristown Medical Center and Overlook Medical Center, ensuring patients and families receive compassionate, individualized care during every stage of their hospice journey.

Compensation

  • Starting pay: $43.00/hour (higher rates based on experience)
  • Mileage reimbursement: $0.725 per mile
  • Per Diem schedule with weekday, weekend, and on-call opportunities as needed by the department

Principal Responsibilities:

  • Clinical Assessment & Hospice Care
    • Perform comprehensive nursing assessments to evaluate patient needs and hospice appropriateness.
    • Develop, implement, and evaluate individualized patient and family care plans.
    • Monitor disease progression and modify interventions based on patient condition.
    • Deliver evidence-based pain and symptom management.
  • Case Management
    • Manage an assigned hospice caseload while providing individualized, patient-centered care.
    • Coordinate care with physicians, hospice aides, social workers, chaplains, therapists, and the interdisciplinary team.
    • Participate in Interdisciplinary Team (IDT) meetings and communicate patient progress.
    • Perform Home Health Aide supervisory visits in accordance with agency standards.
  • Patient & Family Education
    • Educate patients and caregivers regarding:
      • Hospice philosophy
      • Disease progression
      • Medication management
      • Pain and symptom control
      • End-of-life care expectations
    • Adapt education to meet each patient's learning needs.
  • Documentation & Compliance
    • Complete timely and accurate documentation within the Electronic Medical Record (EMR).
    • Maintain compliance with Medicare Hospice regulations and agency standards.
    • Complete admission, revisit, discharge, and case management documentation.
  • Medication & Safety Management
    • Perform medication reconciliation.
    • Monitor medication effectiveness and maintain current medication orders.
    • Promote infection prevention and patient safety.
    • Demonstrate strong pharmacology knowledge and safe medication practices.
  • Quality Improvement
    • Participate in Performance Improvement and Utilization Review initiatives.
    • Mentor and orient new team members.
    • Cross-train to support departmental needs.
    • Attend staff meetings and continuing education opportunities.

Qualifications

Minimum Qualifications:

  • Current New Jersey Registered Nurse (RN) License
  • Associate Degree in Nursing (ASN)
  • Current BLS Certification
  • Valid New Jersey Driver's License with a satisfactory driving record
  • Strong assessment, critical thinking, and communication skills
  • Ability to independently manage a patient caseload
  • Experience using an Electronic Medical Record (EMR)

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN)
  • Minimum one (1) year of experience in:
    • Hospice
    • Home Health
    • Acute Care
    • Subacute Care
  • Experience with hospice case management
  • Knowledge of Medicare Hospice regulations
  • Strong interdisciplinary collaboration skills
