We are seeking a compassionate and skilled Evening Shift LPN to join our team at Morristown Post Acute Rehabilitation & Nursing, a renowned post-acute care center located in Morristown, New Jersey. Responsibilities

• Provide direct patient care to residents in our post-acute setting, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)

• Collaborate with interdisciplinary team members to develop and implement individualized care plans

• Monitor and report changes in resident status to the Registered Nurse (RN) and other healthcare professionals

• Maintain accurate and detailed records of patient care and treatment

• Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction

• Utilize critical thinking and problem-solving skills to address resident needs and concerns

Requirements

• Current LPN/LVN license in the state of New Jersey

• High school diploma or equivalent required
