JOB DESCRIPTION

Atlantic Health System is seeking a dynamic Registered Nurse to join the energetic and innovative team of healthcare professionals as an Inpatient Care Manager on the Care Management Team at Morristown Medical Center. This per diem position requires the candidate to provide services during the day, Monday through Friday, with occasional weekends.

The ideal candidate will be responsible for providing care and services to selected member populations across the continuum of illness, ensuring effective utilization and monitoring of healthcare resources, managing the interdisciplinary team to achieve optimal clinical and resource outcomes, and partnering with the Supervisor/Manager of Case Management to assess, plan, implement, coordinate, monitor, and evaluate services and outcomes to maximize the health and well‑being of the patient.

Weekend-Only

Scheduling Expectations
  • Staff who work weekends only are expected to work a minimum of three (3) weekend days per month.
  • Staff who work weekends only are expected to work a minimum of one (1) weekend shift surrounding a holiday (day before, day of, or day after).
  • Weekend days surrounding holidays will be identified annually by Case Management Leadership. Scheduling assignments will be made on a first‑come, first‑served basis, and holiday assignments will rotate annually to ensure fairness among all staff.
Principal Accountabilities
  • Care Coordination & Patient Advocacy
    • Manage, coordinate, and facilitate care delivery through comprehensive patient and family assessments.
    • Collaborate with multidisciplinary teams to support optimal clinical and psychosocial outcomes.
  • Utilization & Documentation Oversight
    • Conduct daily chart and utilization reviews to ensure appropriate level of care.
    • Maintain accurate, timely, and comprehensive clinical documentation in the electronic medical record (EMR).
  • Interdisciplinary Collaboration
    • Lead and participate in interdisciplinary care rounds.
    • Identify and proactively address quality‑of‑care concerns, barriers to progression of care, and discharge delays.
  • Discharge Planning
    • Formulate, coordinate, and implement individualized discharge plans.
    • Ensure safe, timely, and effective transitions across the continuum of care, including post‑acute services and community resources.
  • Resource & Outcome Management
    • Monitor healthcare resource utilization and length of stay.
    • Partner with the Supervisor/Manager of Case Management to evaluate outcomes and improve care delivery processes.
QUALIFICATIONS Required
  • Current RN license in the State of New Jersey
  • Bachelor of Science in Nursing (BSN) required
  • Minimum 2-3 years of recent acute care nursing experience
  • Strong clinical assessment, critical thinking, and care coordination skills
  • Effective communication and collaboration abilities
  • Experience working with electronic medical records (EMR)
Preferred
  • Prior experience in Care Management, Case Management, Utilization Review, or Discharge Planning
  • Knowledge of payer requirements, regulatory standards, and care transitions
  • Case Management certification (CCM) or ability to obtain within a designated timeframe
