RN - Case Management

AM shift from 8 a.m. to 4 p.m., may require weekend coverage.

The In-Patient Case Manager coordinates patients through the acute care hospitalization, managing all aspects of the discharge planning and transition process. The role involves performing utilization management and quality screenings.

• Coordinate patients through the acute care hospitalization process.
• Manage all aspects of discharge planning and transition processes.
• Perform utilization management and quality screenings.

• 2 years of acute care experience strongly preferred.
• Experience in Case Management and Managed Care required.
• BSN required.
• Current RN license required.
• CCMC certification preferred.
