Responsible for utilizing the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Collaborates with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.
Principal Accountabilities:- 1. Works with interdisciplinary team members to assess and establish plans of care.
- 2. Collaborates with all members of the health care team with a focus on promoting evidence based care within a healing culture for the patient population served.
- 3. Mentors field staff in all aspects of caring for patients.
- 4. Ensures current proficiency of knowledge regarding Medicare COP's and Joint commission standards..
- 6. Performs other related duties as assigned.
