Job Summary:
Responsible for coordinating patient care within the acute hospital setting, focusing on discharge planning, transition management, and utilization oversight. Ensures comprehensive case management to promote quality outcomes and efficient resource utilization.
Responsibilities:
- Coordinate patients through the acute care hospitalization process.
- Manage all aspects of discharge planning and transition of care.
- Perform utilization management and conduct quality screenings.
- Collaborate with interdisciplinary teams to ensure patient needs are met.
- Document patient progress and compliance with care plans.
- Facilitate communication between patients, families, and healthcare providers.
Qualifications:
- Minimum of 2 years of acute care experience preferred.
- Active RN license required.
- BSN degree required.
- Strong knowledge of case management and managed care principles.
- Experience with inpatient case management preferred.
- Certification such as CCMC is a plus.
