Pay Range: ($61.23-$67.43)/hourly ($2,296.30-$2,528.50)

Job Summary
  • Performs patient assessments to determine appropriateness for hospice care, identify learning needs, and evaluate progress toward goals.
  • Develops and updates individualized plans of care for patients and families.
  • Ensures appropriate utilization of services while maintaining visit productivity standards.
  • Demonstrates knowledge of insurance reimbursement guidelines and hospice regulations.
  • Completes discharge assessments accurately and in a timely manner.
  • Performs laptop transfer of patient data twice daily.
  • Completes admission assessments and documentation transfers within 24 hours.
  • Completes re‑visit documentation and required data transfers promptly.
  • Uses age‑appropriate teaching methods for patients and families.
  • Evaluates and revises teaching plans as necessary.
  • Documents patient education tools and teaching methods utilized.
  • Assumes responsibility for case management activities.
  • Develops, implements, and evaluates evidence‑based plans of care.
  • Revises care plans according to patient condition and identified needs.
  • Prioritizes patient care needs and scheduling to accommodate patient requirements.
  • Demonstrates effective interdisciplinary communication and collaboration.
  • Maintains a safe environment for patients and co‑workers.
  • Adheres to infection control practices and reports infections appropriately.
  • Demonstrates knowledge of pharmacology and safe medication management.
  • Routinely reconciles medications and maintains current medication orders.
  • Attends monthly staff meetings and participates in Performance Improvement (PI) activities and committees as requested.
  • Reviews health records and gathers data to support PI activities.
  • Attends Utilization Review (UR) meetings as requested.
  • Provides orientation and mentorship to new employees.
  • Cross‑trains to learn new skills and demonstrates willingness to teach others.
  • Assists with agency growth and business development initiatives.
  • Accepts additional assignments and responsibilities as requested.
  • 11a‑7p and every other weekend Must have previous GIP exp. within a hospital setting.
Required Qualifications
  • Must have two (2) years of experience as RN in home hospice environment.
  • Experience using an EMR, excellent critical thinking and communication skills.
Education
  • Associate's (ASN) degree
  • OR
  • Diploma with RN License in New Jersey
Certifications & Licensure
  • Current New Jersey RN License
  • Must have valid New Jersey driver’s license.
  • BLS.
