Position Overview

The RN is responsible for the full delivery of the plan of care for each hospice patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity and improve the quality of care delivered.

Responsibilities
  • Make assigned patient care visits in accordance with physician orders.
  • Complete supporting documentation accurately and timely, including visit report, OASIS assessment, plan of care and revisions, medication profile, progress notes, recertification, discharge/transfer summary, etc.
  • Attend meetings for all patients assigned.
  • Participate and offer input regarding each patient’s care and progress toward individual patient goals.
  • Understand and adhere to all agency policies, procedures, and processes (administrative, personnel, clinical, etc.).
  • Seek guidance and clarification when uncertainty or confusion arises.
  • Establish and maintain an effective and efficient visit route.
  • Provide services within the scope of practice as defined by state laws governing the practice of nursing, initiate and coordinate the plan of care, and collaborate with patients/families, attending physician, and other members of the interdisciplinary team in providing patient and family care daily.
Qualifications
  • One (1) year of clinical RN experience.
  • Current RN license in the state where services are rendered.
  • Current CPR certification.
  • Driver’s license, vehicle insurance, and dependable vehicle or public transportation access.
  • Reliable commute to multiple patient residences daily in the assigned geographic area.
