This position carries the same pay grade and qualifications as the Developmental Services Community Integration Specialist (CIS) and is responsible for fulfilling all requirements and responsibilities of the CIS role. In addition to providing direct support services, the Community Transportation Specialist transports consumers using an agency vehicle and ensures all transportation activities are conducted safely, professionally, and in accordance with agency policies.
The Community Transportation Specialist will be assigned transportation responsibilities by the DS CIS/Employment Coordinator and will document all transportation services in the electronic medical record (EMR), including the times consumers are transported. When transportation services are not needed, the Community Transportation Specialist will provide one-to-one or one-to-two direct support services to consumers as assigned.
An agency vehicle will be provided for use during work hours. Staff will receive ongoing training, supervision, and support. A valid driver's license is required for this position.
Essential Duties and Responsibilities Provide safe, reliable, and timely transportation for consumers to employment, appointments, community activities, and other approved destinations.
Support consumers in developing practical life skills, self-advocacy, socialization, adaptive skills, and community engagement while in the community.
Fulfill all responsibilities and requirements of the Developmental Services Community Integration Specialist (CIS) position.
Provide one-to-one or one-to-two direct support services when transportation services are not required.
Maintain accurate and timely documentation of transportation services and consumer support activities in the electronic medical record (EMR).Record transportation details, including the time consumers are transported in agency vehicles.
Conduct routine vehicle inspections and perform basic maintenance checks to ensure vehicle safety and operational readiness.
Maintain accurate vehicle logs, including mileage, fuel usage, and maintenance records.
Communicate effectively with consumers, families, coworkers, and supervisors regarding schedules, transportation needs, and service delivery.
Assist with loading, unloading, and lifting equipment or supplies related to transportation activities as needed.
Follow all traffic laws, agency policies, and safety regulations to ensure the safety of consumers, staff, and the public.
Participate in required trainings, meetings, and professional development opportunities.
