Transitional Housing Specialist (Full-time Hybrid, North Carolina Based) (Healthcare)
The purpose of the Transitional Housing Specialist is to foster an atmosphere of support and collaboration amongst housing providers, Alliance Health staff and network providers to promote access to short-term bridge housing programs that provide the intensity, education, and skill building for members to offer alternatives to congregate care and step down from hospitals and crisis facilities.
This is a full-time hybrid opportunity. There is no expectation of coming into the office routinely, however, the selected candidate must be available to report onsite to the nearest Alliance Office for business meetings as needed. The successful candidate will also be required to travel as needed throughout Alliance catchment area and surrounding counties (including ones outside of Alliance's catchment area) to meet with members, providers and/or other community stakeholders.
Responsibilities& Duties Ensure Compliance Standards are Met
- Monitors transitional housing programs and providers to ensure compliance with the required elements of program and services outlined in the Alliance contract
- Monitors transitional housing programs to ensure adherence with acceptance and denial of applicants under the program guidelines
- Manage the transitional housing programs referrals for accuracy and appropriateness
- Maintain accurate, complete, and timely records and reports ensuring compliance with funding and contractual obligations
- Coordinate with transitional housing staff to meet goals and objectives for housing participants for immediate and long-term housing plans
- Take initiative in problem-solving and utilize resources, including team members and community resources, to assist in addressing challenges
- Provide technical support and training to TCL staff, and Providers, regarding the spectrum of Housing options in the community and developing tools to assist in fostering relationships with Housing Vendors
- Build relationships with referral sources, including healthcare programs, county agencies and state facilities
- Enhance collaboration with community partners to expand participant services
- Develop streamlined administrative processes to ensure compliance and sustainability
- Implement best practices in referral process, program documentation, and tracking progress
- Travel between Alliance offices, attending meetings on behalf of Alliance, participating in Alliance sponsored events, etc. is required
- Travel to meet with members, providers, stakeholders, etc. is required
Bachelor's degree from an accredited college or university and three (3) years of experience in Human Services, with at least two (2) years of experience in housing required.
Knowledge, Skills, & Abilities- Exceptional interpersonal skills, highly effective communication ability, and the propensity to make prompt independent decisions based upon relevant facts
- Problem solving and conflict resolution skills
- Strong knowledge of behavioral health and intellectual/developmental disabilities
- Strong knowledge of homeless system and housing principles, guidelines, laws and concepts
- Knowledge of housing market trends
- Knowledge of all guidelines and regulations in the Americans with Disability Act
Employment for this position is contingent upon a satisfactory background and MVR (Motor Vehicle Registration) check, which will be performed after acceptance of an offer of employment and prior to the employee's start date.
Salary Range$61,439- $78,335/ Annual
Exact compensation will be determined based on the candidate's education, experience, external market data and consideration of internal equity
