Healthcare Provider - Nurse Practitioner​/Physician Assistant

• Evaluating and treating patients following care standards
• Coordinating services such as referrals, hospital and SNF liaison, and home health
• Leading and participating in team-based care activities
• Ensuring timely medical documentation and compliance with policies
• Engaging in quality improvement and growth initiatives
• Participating in on-call programs and community outreach

• Board-certified Nurse Practitioner or Physician Assistant with active license
• 2-5 years of relevant experience preferred
• Excellent communication and interpersonal skills
• Ability to adapt to value-based care models
• Basic computer proficiency and certification for prescribing medications
• Willingness to participate in TB screening and maintain compliance with licensing standards

• Active DEA, Medicare, and Medicaid provider numbers
• Experience managing Medicare Advantage panels and understanding of Medicare guidelines
• Knowledge of HEDIS quality indicators
