Chen Med is seeking a compassionate Medication Technician to support safe, accurate medication administration for seniors with complex chronic conditions. In this role, you will prepare and administer medications per provider orders, document in the EHR, monitor vital signs and side effects, and promptly report changes in patient status. Working closely with physicians, nurses, and care teams, you'll help coordinate refills, support medication education, and reinforce adherence.
This position is highly patient-facing, focused on building trust, delivering respectful care, and supporting Chen Med's mission to improve outcomes and quality of life for vulnerable seniors in our value-based, team-centered primary care model.
Responsibilities
- Prepare and administer medications to patients according to provider orders and state regulations
- Verify medication orders, dosages, and patient identity to ensure safe administration
- Document medication administration and patient responses in the electronic health record
- Monitor and record vital signs, side effects, and changes in patient condition
- Communicate promptly with nurses and providers about clinical concerns or adverse reactions
- Assist with medication reconciliation, refills, and inventory management
- Educate patients and caregivers on medication schedules, purposes, and adherence
- Follow infection control, safety, and HIPAA standards in all patient interactions
- Support the care team in coordinating appointments, treatments, and follow-up
