The RN Case Manager coordinates and manages patient care from admission through discharge. • Coordinate and manage patient care throughout the continuum of services, ensuring timely and appropriate care.
• Develop individualized care plans in collaboration with patients, families, physicians, and other healthcare providers.
• Serve as a liaison between the patient, family, and healthcare team to ensure clear communication and understanding of care plans.
• Monitor and evaluate patient progress, adjusting care plans as necessary to achieve optimal outcomes.
• Conduct assessments to identify patient needs, resources, and barriers to care.
• Facilitate discharge planning, ensuring patients have the necessary follow-up care, resources, and support upon leaving the hospital.
• Advocate for patient needs, ensuring they receive the appropriate level of care and services.
• Collaborate with social workers, therapists, and other healthcare professionals to provide comprehensive care.
• Ensure compliance with regulatory requirements, hospital policies, and best practices.
• Participate in quality improvement initiatives and case reviews to enhance patient care delivery.

Candidate requirements

• Current Registered Nurse (RN) license in Washington.
• Bachelor of Science in Nursing (BSN) is preferred;
  Associate Degree in Nursing (ADN) is considered with relevant experience.
• Minimum of 2 years of clinical nursing experience, preferably in case management, discharge planning, or a related field.
• Strong understanding of care coordination, discharge planning, and case management processes.
• Excellent communication, organizational, and problem-solving skills.
• Ability to work independently and as part of a multidisciplinary team.
• Certification in Case Management (CCM) preferred.
