Licensed Vocational Nurse

Columbia Crest Center in Moses Lake, WA is seeking Licensed Vocational Nurses to provide compassionate, resident-centered care in our skilled nursing and rehab facility. In this team-oriented role, you will administer medications and treatments, monitor resident conditions, support rehab goals, and document care in our EHR. You'll collaborate closely with RNs, CNAs, therapists, and physicians to deliver personalized care plans that promote recovery, comfort, and quality of life.

Responsibilities

• Provide direct nursing care to residents under RN and provider direction
• Administer medications and treatments per physician orders and facility policy
• Monitor, document, and report changes in resident condition using EHR systems
• Participate in care planning and interdisciplinary team meetings
• Perform wound care, vital signs, and basic assessments within LVN scope
• Support rehab goals through coordinated care with therapy staff
• Educate residents and families on care plans and health needs
• Follow infection control, safety, and quality standards
• Supervise and support CNAs to ensure high-quality resident care
• Document care accurately and timely to meet regulatory requirements
