Licensed Vocational Nurse

Columbia Crest Center in Moses Lake, WA seeks a compassionate Licensed Vocational Nurse to provide direct nursing care in a skilled nursing and rehab setting. The LVN will deliver medications and treatments, support short-term rehab and long-term care plans, and closely monitor resident status. Working in a team-oriented, resident-focused environment, you'll collaborate with RNs, therapists, and other staff to provide individualized, person-centered care.

Responsibilities

• Provide direct nursing care to residents, including medication administration, treatments, and monitoring vital signs.
• Support short-term rehab and long-term care plans under RN and physician direction.
• Document care accurately in electronic health records and follow clinical protocols.
• Collaborate with an interdisciplinary team to support individualized, person-centered care.
• Educate residents and families on care plans, treatments, and health maintenance.
• Respond to changes in resident condition and report promptly to supervising clinicians.
• Assist with admissions, discharges, and care transitions.
• Ensure compliance with infection control, safety, and regulatory standards.
• Support resident comfort, dignity, and quality of life in daily activities.
• Participate in ongoing training, skills development, and quality improvement efforts.
