Full-Time Administrative LPN
Monday - Friday 8 am to 4 pm
DUTIES:
- Assist in creation of plan of care for all new individuals served
- Conduct supervisory visits and document results
- Complete and update health, risk and safety assessments using the Health Risk Screening Tool (HRST)
- Review of data tracking and documentation
- Provide staff training and oversight
- Supervision of LPN Staff
- Coordinate medical visits for clients
- Fill in on medication passes as necessary
- Other duties as assigned
