Active DEA, BLS required

Credentialing Timeline: 6090 days

COMPENSATION & BENEFITS

Base Salary: $275K Annually

Incentive Bonus Structure: $50K Sign-on bonus; RVU - $41.75 Threshold at 500 a month.

Relocation Assistance: $25K (Submitted after relocation)

CME Allowance: $6000 for 5 days

PTO & Holidays: 25 Days

Provide preventive, chronic, and acute care services
